Just like in English interrogative pronouns can be used in exclamations.
¡Qué imbécil es!
= What an idiot!
¡Qué bonito!
= How nice! (What nice!)
¡Qué calor que hace!
= How hot it is! (What hot it does!)
¡Quién lo habría pensado!
= Who would have thought!
¡Cómo has cambiado!
= How you have changed!
¡Cuánta gente hay en la calle!
= How many people are on the street!
